Program Rules

Join the Program:

Live Healthy Rewards is a free program, but you need to join every calendar year to participate. You can join right now online, or you can call 1-866-215-0062 (TTY users call 711) to join by phone.

Complete Your Health Survey:

Once you join, you finish your enrollment in the Live Healthy Rewards program by completing your Health Survey, a series of 12 general questions about your health. Once the Health Survey is complete, you are eligible to start earning Rewards. If you have a completed Health Survey for the current calendar year on record, you may skip this step and you are finished with your Rewards program enrollment. You will need to complete a Health Survey every calendar year to complete your enrollment for that program year.

Track Your Progress – Health Scorecard:

As you complete eligible health actions, they are recorded on your Health Scorecard. The Scorecard is available for review online in your Live Healthy Rewards section. You can see which health actions you have completed and which ones you may still need to do. You may also call Live Healthy Rewards at 1-866-215-0062 to review your Scorecard over the phone.

Health Action Completion:

You can earn $50 in Rewards dollars once you have completed five different eligible health actions during a single calendar year. Each completed health action will count only once toward your Rewards, regardless of how many times you may complete that same action in a calendar year. Please allow at least 60 days for automatic updates. (Most claims are processed within 60 days; however claim processing varies and in some cases may take longer.) You must report your health actions before December 31, 2014 to receive credit.

Receive Your Live Healthy Rewards Card:

You will be mailed a Live Healthy Rewards card in the amount of $50, once you have completed five different eligible health actions. Your prepaid card can be used to spend your Rewards dollars (at select retailers) on a wide variety of products. Examples include over-the-counter health products, such as vitamins, supplements, allergy medicine, cold and flu remedies, pain relievers, or for food and beverage products offered at our participating food retailers.

Select Retailers:

There is a list of national retailers that will accept your Rewards card. (The card will not work at retailers outside of the approved Live Healthy Rewards retail list.) The card may be used online subject to retailer restrictions.

Rewards Card Expiration:

You can earn 2014 Rewards beginning on January 1, 2014. The card, and all remaining Rewards dollars, will expire one year from the date the card is issued. Example: If you are issued the Rewards Card on June 15, 2014, your card will expire on June 30, 2015. You will need to use the Rewards dollars before the date of expiration on your card.

  • Q. Is there a charge or fee to be part of the Live Healthy Rewards program?
  • A. No. It's a free program for plan members. You need to join to participate and receive the Rewards dollars. You can join online, or you can call 1-866-215-0062 (TTY users call 711) to join by phone. You will need to join and complete a Health Survey each calendar year you participate in the Live Healthy Rewards program.

  • Q. What do I do to earn Rewards?
  • A. It's simple. Complete your enrollment by taking a short Health Survey. Once completed, you are eligible to begin earning Rewards. If you have a completed Health Survey for the current calendar year on record, you may skip this step and you are finished with your Rewards program enrollment. You can earn $50 in Rewards dollars for completing five different eligible health actions. The qualifying health actions include: annual flu shot, annual physical, glaucoma test, cholesterol screening, colorectal cancer screening, breast cancer screening (for women only), and prostate cancer screening (for men only). There's no need to call us; your claim notifies us when you've earned a reward. If you joined the program last year, you may be eligible for rollover credit. See "What if I completed my health action last year?"

  • Q. Is there a limit to how much I can earn in a year?
  • A. Yes. You can earn $50 for completing any five different eligible health actions during a single calendar year. Each completed health action will count only once toward your Rewards, regardless of how many times you may complete that same action in a calendar year.

  • Q: What if I completed my health action last year? Will it carry over this year?
  • A: Most health actions will need to be completed each calendar year to qualify for credit (See below for exceptions). Example: if you got your flu shot in November 2013, your credit will not carry over to 2014. You will need to get another flu shot to qualify for 2014 (subject to your doctor's approval and medical need). Live Healthy Rewards is an annual program that encourages members to take care of their health all year long. Therefore, it may take several months to complete your health actions and qualify for Rewards credit. Please check with your doctor to determine what is right for you.

    Exceptions to annual requirement include:

    • Breast Cancer Screening - this screening is valid for two (2) years from the date of service
    • Colorectal Cancer Screening - there are two (2) exceptions:
      • If you received a Flexible sigmoidoscopy, this screening is valid for five (5) years from the date of service.
      • If you received a Colonoscopy, this screening is valid for ten (10) years from the date of service.

    If you have questions about credit for 2013 health actions completed or 2013 eligibility, please contact the 1-866-215-0062 (TTY users call 711) to review your eligibility.

  • Q. What if I already had a flu shot?
  • A. If you received a flu shot this year, then you can get credit for it. You can self-report the completion of your flu shot health action in your Health Scorecard. If you don't see credit for a health action you completed or you have further questions, please call Live Healthy Rewards at 1-866-215-0062. Remember, if you completed your flu shot anytime in the 2013 calendar year, your credit will not carry over to 2014.

  • Q. Why do I need to complete another Health Survey if I already completed a survey last year?
  • A. The Health Survey provides information on your current health and wellness. Enrollment into Live Healthy Rewards includes completion of a Health Survey each calendar year to be eligible for the program. By completing a new Health Survey for this calendar year, you will be eligible to begin earning Rewards by taking positive health actions as outlined in the summary below.

  • Q. What if I completed a health action with another plan?
  • A. If you completed a health action with another plan, you can get credit for it by submitting proper documentation from either you or your provider. Proper documentation (also known as acceptable proof of service) includes a copy of lab screenings, results, tests, and/or an Explanation of Benefits for the service. Proof of service must include your Name, Date of Birth, date of service and the name of the health service to qualify for credit. Upon receipt of your supporting documents, we will review the information to determine qualification. If we determine that the supporting documentation meets the stated requirements, we will award your credit accordingly. You will need to allow 3-4 weeks for internal review and determination. Please call Live Healthy Rewards at 1-866-215-0062 for assistance with this process.

  • Q. What if I do not see a health action I already completed?
  • A. If you completed a health action this year in our plan, you can get credit for it. Please allow at least 60 days for automatic updates. (Most claims are processed within 60 days; however claim processing varies and in some cases may take longer.) If you don't see credit for a health action you completed or you have further questions, please call Live Healthy Rewards at 1-866-215-0062.

  • Q. Are the Rewards dollars paid in cash?
  • A. No. Your Live Healthy Rewards are credits on a special prepaid card (similar to a retail gift card). You take the card to participating retailers (see list below) and use it to buy over-the-counter health products and healthy food.

  • Q. Where can I use my card and spend the Rewards I've earned?
  • A. Your card is accepted at a select list of national retailers only. The Rewards card may be used online subject to retailer restrictions. Retailers include:

  • Q. Can I use my Rewards card to buy anything at any of the select retailers?
  • A. No. Rewards dollars are for over-the-counter health products and health food, such as vitamins, supplements, allergy medicine, cold and flu remedies, oral hygiene products, sleep aids, foot care, first aid, pain relievers, or for food and beverage products offered at our participating food retailers.

  • Q. How do I know how many health actions I have completed?
  • A. When you sign up for the Live Healthy Rewards program, you also gain access to MemberLink, the Member Portal for your health plan. In MemberLink, click on the Live Healthy My Rewards tab and then go to "My 2014 Health Scorecard." Your account will show the health actions you have completed and which ones you may still need to do.

  • Q. How do I check my balance or transaction history on my Rewards card?
  • A. If you have any questions about transactions or balance, please call the customer service number listed on the back of your Rewards card.

  • Q. What if I have lost my Rewards card?
  • A. If you need to report your Rewards card as lost or stolen please call the card vendor, OmniCardTM, at 1-855-253-7079, 24 hours a day, 7 days a week (TTY users call 711) or visit the OmniCardTM website at www.omnicard.com (by clicking on this link you will be leaving our website).

Prostate cancer
The American Cancer Society recommends that men make an informed decision with their healthcare provider about whether to be screened for prostate cancer. Research has not yet proven that the potential benefits of testing outweigh the harms of testing and treatment. The American Cancer Society advises that men should not be tested without learning what is known about the risks and possible benefits of testing and treatment. Starting at age 50, men should talk to their doctor about the pros and cons of testing so they can decide whether testing is the right choice for them. Men should have this talk with their doctor starting at age 45 if they are African American or have a father or brother who had prostate cancer before age 65.
